If you have lost your athletic forms packet, all the forms are below for you to print (or you can stop by SW to pick one up.) Please keep in mind that the forms packet must be turned in prior to the first practice in order to participate in athletics. The sports fee is due at the end of the first week of practice. Athletic physicals are good for three calendar years from the date of the physical. Once the physical has expired, another must be conducted with a form on file in the athletic office. A downloadable form is available below. If you have any questions regarding the expiration date of your student's physical, please contact Jon Longnecker in the athletic office at (952) 556-0040. 2012-2013 Forms: Sports Qualifying Physical Exam – Schedule a physical with the family doctor (preferably as soon as possible) and have him or her fill out this form. This form remains on file for three years. After three years, you must have this done again. *Many people get their physical as a freshman, and, as a result, need another physical prior to their senior year. |
